Where do I submit documentation for my absence?

  1. Submit documentation online at https://forms.as.uky.edu/general-chemistry-excused-absences which will have all the information needed to have your absence excused.
    • If you missed lab, exceptions are set based upon the information on the form.
    • No assignments for an experiment can be completed until the student performs the lab experiment during the make-up lab time.
  2. Be specific (i.e. class on 3/4, recitation on 3/5) about what was missed.
  3. Examples of appropriate documentation include:
    • a note from a doctor,
    • letter from coach for student athletes,
    • letter from Dean for participation in University sponsored event,
    • obituary notice and/or funeral program.
  4. Include any requested extensions in the comments (such as Achieve or PlayPosits)
